Axial Exchange Helps Direct Under-Served Patient Population to Local Safety Net Clinics

Axial Exchange and the Capital Care Collaborative (CCC) announce the creation of an interface that provides case managers with real-time admission, diagnosis, and discharge information with which to better serve their patients as they transition from emergency room care to the ambulatory clinics that serve them.

“We believe this will deliver a smoother transition of care for the community of patients served by the Collaborative. We thank the CCC for taking a leadership role in participating with Axial on this project and setting a standard for other communities to follow. said Joanne Rohde, CEO, Axial Exchange Inc. “This is a great step toward lowering unnecessary hospital readmissions.

According to Lisa Rowe, Director of the CCC, the hospital interface allows partner organizations and case managers to see where uninsured patients are going for care and how often, the prevalence of specific disease types that result in emergency room visits, and who the patients are that use the emergency room for non-emergent care. Armed with this information, Rowe states, we can connect patients to primary care and provide targeted intervention strategies such as patient education regarding where to go for care and care management for patients with chronic conditions.
